Terms of Service

The Sensory Coach Inc. (hereafter “The Sensory Coach”) is pleased to offer personalized and effective services for all clients (hereafter “Client”). As a Client, you must review and agree to the following terms and conditions and discuss any item(s) that are unclear with a team member of The Sensory Coach.

  1. Cancellation Policy

The Sensory Coach has a 24-hour cancellation policy. If you miss an appointment, or cancel with less than 24-hour notice, you may be subject to a $25 no show fee.

  1. Description of Services

Depending on your location, you may not be eligible for all services. The Sensory Coach provides the following services for eligible clients:

  • Screenings and Consultations
  • Parent/Individual Coaching
  • Couples/Family Coaching
  • Small Groups
  • Occupational Therapy Evaluations and Treatment (see ‘Service Limitations’ below)
  • Professional Consultations, Mentoring, and Collaborations
  • Academic and Professional Learning Instruction
  1. Delivery of Services

Apart from occupational therapy services (see ‘Service Limitations’ below), all services provided by The Sensory Coach are available in the following formats:

  • Online or via phone
  • Home/community locations
  • Clinic-based
  1. Service Limitations

At this time, occupational therapy service delivery is limited to the states of Colorado, Georgia, and Illinois.

  1. Health and Safety Procedures

For all in-person services, appropriate health and safety procedures will be maintained without exceptions. These include the following:

  • All individuals involved must be in good health and fever free for 24 hours prior to in-person appointments
    • Sessions will be terminated immediately if any individual involved shows signs of illness before or during the appointment
    • Single use or sanitization procedures for all objects
    • Use of masks, gloves, and other personal protective equipment whenever a coach/therapist deems it necessary
  1. Fees for Services

Services are billed by the hour, half-hour, and quarter-hour, and are due either at the time of service or prior to subsequent services. Payment arrangements (see ‘Payment Plans’ below) must be made prior to provision of any additional services. If no prior arrangements have been made, full payment will be due prior to receiving any further services.

  • Pre-payment plans
  • Monthly payments
  • Pay at time of service
  • Invoice billing (default)
  1. Insurance

The Sensory Coach is an out-of-network therapy provider in the states of Colorado and Georgia. Please check with your insurance company for benefits. You are personally responsible for paying the full cost of therapy. Superbills (forms to submit to insurance for reimbursement) are available upon request.

  1. Program Changes

The Sensory Coach reserves the right to change, modify or cancel any programs as considered necessary. Any such change will be made in consultation with you.

  1. Indemnification

At times, a program with The Sensory Coach may present certain inherent risks. By participating in any service provided by The Sensory Coach, you expressly assume the risks of the program and take full responsibility for all personal decisions made before, during, and after the provision of services.

  1. Confidentiality

The Sensory Coach, and its employees, agents, or representatives will not at any time or in any manner, either directly or indirectly, divulge, disclose, or communicate in any manner, any information of yours that is private and protected, including health or financial information, without prior express written consent. Any identifying information obtained from you will be treated as privileged and confidential and will not be released or revealed to any person without expressed written consent. Confidentiality will be upheld even after the term of any program or agreement has expired.

  1. Mandatory Reporting

The Sensory Coach therapists and coaches are mandatory reporters and are therefore required by law to report observed or suspected abuse or neglect of minors. Furthermore, persons believed to be at imminent risk for committing violence or attempting suicide will be reported to a person or persons reasonably able to prevent or lessen the threat, including the target of the threat.

  1. Program Waiver

Prior to beginning a program with The Sensory Coach, you are required to sign a participation waiver. You must agree to waive any and all liability in the unlikely event you experience injury or illness resulting from participation in a program with The Sensory Coach. You assume responsibility for any personal, medical, or financial effects arising from participation in anything related to the program.

  1. Agreements

By entering into a Therapy or Coaching Agreement either through signing a Therapy or Coaching Agreement with The Sensory Coach or paying any amount to your Sensory Coach, you agree to abide by the terms and conditions as set out above.

  1. Work Product Ownership

Any copyrightable works, ideas, discoveries, inventions, patents, trademarks, products, or other information (collectively the “Work Product”) used or developed in whole or in part by The Sensory Coach in connection with the Services will be the exclusive property of The Sensory Coach. Upon request, The Sensory Coach will execute all documents necessary to confirm or perfect the exclusive ownership of The Sensory Coach to the Work Product.
